Certificate of Independence

Important Notice: For both Evaluation Methods and the Testing Method, applicants must attach a "Certificate of Independence" to their online application. (Please DO NOT mail it to DCA staff.) A Certificate of Independence is requested from the Evaluation or Test Agency (NOT the Validation Entity) certifying that the Test or Evaluation Agency is independent from the Manufacturer. Name it "Certification_of_Independence", convert it to PDF format and attach it to the application at any of the Upload points (Test or Evaluation Report or Installation Instructions).

*All applications using Evaluation by an Architect or Engineer method must send in a signed and sealed hard copy of the evaluation report AND a copy of the online payment receipt showing the FL number. Please send both of these to:
